Referring Providers:
Thank you for referring your patient to Restore and Grow Nutrition and Sleep. I am in network with BCBS and Aetna, and accept private pay (see Insurance page for rates).
To refer a patient, please fax me the following:

demographics and insurance information
-
most recent office notes
-
growth charts (WHO 0-24 months weight, length, and weight-for-length OR CDC 2-20 years weight, height, and BMI)
-
any relevant labs.
Please also provide the patient (or parent/caregiver) the link to set up a discovery call with me. I will also contact them, but if they book a discovery call this will streamline the intake and scheduling process.
